and have received a copy of the rules. I am aware
that if the team wish to withdraw from the league
we will be required to give two weeks notice of
our intent to do so otherwise we will be liable
to pay two weeks match fees.
I accept full responsibility on
behalf of my team to fulfil and pay for all fixtures
as required. I confirm that I am 16 years of age
or over.
Returning this completed form electronically to us
constitutes acceptance of the terms & conditions
of the League.
By entering into a league your team will automatically
be affiliated to the EIFF which includes public
liability insurance. This covers any accidents you
may have whilst playing in the league, which may be
the result of our negligence. Claims are limited to €16,500,000.

at our own risk in any of our league organised events
and we cannot be held responsible for personal
injury sustained whilst participating in the leagues.
I understand cover for personal injury is our teams
responsibility to organise. It is the responsibility of
each player to arrange either their own insurance cover
or a teams insurance cover that will cover them for
any injury sustained whilst playing in their leagues.
